01 Acceptance of Terms
These Terms and Conditions ("Terms") govern your access to and use of the Shore Parkway Physical Therapy, PC website (the "Site"). By accessing or using the Site, you agree to be bound by these Terms. If you do not agree, please do not use the Site.
These Terms apply to all visitors and users of the Site, whether or not you become a patient of our practice.

You may use the Site for lawful purposes only and in accordance with these Terms. You agree not to:

A clinical relationship is established only after you become a patient of the practice through our intake process and an in-person consultation.

04 Appointments & Cancellations
Appointment requests submitted through the Site are not confirmed until our office contacts you to schedule. We typically respond within one business day.
We ask that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. Late cancellations or missed appointments may be subject to our cancellation policy, which will be provided to you at intake or upon request.
05 Text Messages & Electronic Communications
If you opt in to receive text messages from us, you consent to receive non-marketing texts related to appointment scheduling, appointment reminders, and follow-up communication about your care.
Our messaging is sent through a HIPAA-compliant platform operated under a Business Associate Agreement. Because standard SMS travels over external carrier networks that are not end-to-end encrypted, your opt-in serves as your consent to receive appointment-related communications by text, which may include limited information about your care.
Message frequency varies. Message and data rates from your carrier may apply. Reply STOP to opt out or HELP for help at any time.
06 Intellectual Property
All content on this Site — including text, graphics, logos, images, icons, and design — is the property of Shore Parkway Physical Therapy, PC or its licensors and is protected by United States and international copyright, trademark, and other intellectual property laws.
You may view, download, and print Site content for your personal, non-commercial use only. Any other use — including reproduction, modification, distribution, public display, or republication — requires our prior written permission.
07 User Submissions

11 Indemnification
You agree to indemnify, defend, and hold harmless Shore Parkway Physical Therapy, PC and its owners, employees, contractors, and affiliates from any claims, damages, liabilities, costs, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms, your use of the Site, or your violation of any rights of a third party.
12 Privacy & HIPAA
Your use of the Site is also governed by our Privacy Policy, which explains how we collect, use, and protect information you provide to us.
As a licensed healthcare provider, our handling of Protected Health Information is governed by HIPAA and described in our separate Notice of Privacy Practices, which is provided to all patients at intake. Our practice management and patient communication systems are operated through HIPAA-compliant platforms covered by Business Associate Agreements.
13 Governing Law & Jurisdiction
These Terms are governed by and construed in accordance with the laws of the State of New York, without regard to its conflict-of-law principles.
Any dispute arising out of or relating to these Terms or your use of the Site shall be brought exclusively in the state or federal courts located in Bronx County, New York, and you consent to the personal jurisdiction of those courts.
14 Changes to These Terms
We may update these Terms from time to time. When we do, we will post the updated version on this page and update the "Last Updated" date at the top. Your continued use of the Site after changes are posted means you accept the updated Terms.
15 Severability
If any provision of these Terms is found to be unlawful, void, or for any reason unenforceable, that provision shall be deemed severable and shall not affect the validity and enforceability of the remaining provisions.
